Why is the middle wheel smaller than the rest- there for is 1/2" space between the botto of that wheel and the ground?
This configuration is designed in this way, and it is grounded under load-bearing conditions.

Will it fit a 7.3 idi diesel block widest bolt pattern is 18 inches if so I'll buy it
However, the critical issue is the adapter plate diameter and arm reach clearance, not just bolt spacing. For a 7.3 IDI (heavy iron diesel), it will also be at the upper limit of safe handling even though it’s rated 1500 lb. if this answer does not resolve the current issue, please contact us again for assistance.

will probably fit, but not guaranteed for a 7.3 IDI without checking the bolt pattern on the stand head. This VEVOR stand has 4 fully adjustable arms on a rotating plate, designed to fit “most engines” and common V8/V6 patterns. The arms slide to different bolt-hole positions, so an 18-inch widest bolt pattern is within typical adjustability range for this style of stand.
