What amperage does the breaker require for the 40 AMPS? Some chargers need 60 AMP breaker

This product requires a 50A circuit breaker.

Is there temperature protection in the 14-50 plug? For lose connection overheating/melting. Especially at 40 amps.

14-50 plugs do not have temperature protection. If the socket is old and loose, it may melt. If the socket is old, it needs to be replaced.
