It comes as shown. There are slots where your old seal came out of. This slides right back into those two slots. My new seal slid into the slots very easily.

This is the seal only, it slides into the channels on the bottom of the door, after removing the original seal.

No the metal plate that attaches to the bottom of the garage door does not come with this seal. It would need to be purchased separately and is needed to use this seal.
