Does it have bearings under the adjustment screw?

For heavy loads adjust the height first, with lighter loads the jack screw will lift the load fine.

Can you lock the head /holder so it doesn't spin or rotate once you adjust the height to where you want it?

No you can't, the head will spin around in the looked. Even with that, I'd say it was still a good investment. Very sturdy, never had any issues with mine.
