How wide is the space on the fixed, top shelf-From the top of the unit to the shelf? Thanks.

Each shelf is made to fix them in the position you need/want. I currently have the top shelf set to hold the height of family-size cereal boxes. Alternatively, It can be set to hold the height of one layer of canned goods.

No, this does not have reserved caster holes. It is recommended that you just buy one with casters.

Can I order extra adjustable shelves for this product?

If you are looking for something long term I would look elsewhere. Mine came with every single piece needed adjustments for all the dents just to fit pieces together, paint was cracked and peeling and I have yet to finish banging it together. I only ordered it to hold our business saftey gear that is why I didn't send back.
