how do you tighten the hand held shower that attaches to the wall. There is no way to tighten it to the wall

The handheld shower bracket is secured to the wall with mounting screws behind the cover plate. If it feels loose, remove the decorative cover, tighten the mounting screws, and reinstall the cover.
