"run out" is undefined, but the 3 point accessory holder with tool to tighten holds every one of several bits I have used perfectly true and straight.
The issue I've encountered is overloading the bit end causing it to slow down. If you do this (this is speed control and pressure, both under your control), the snake inside the extension will bind and twist. Do not do this, or back off immediately! But the business end is totally true.

Make sure cord is tight to handle and everything is good and tight and it should not wobble. I had that problem but didn’t check tightness!

I have not tried to measure the runout on this handpiece. I will observe that qualitatively, it's no worse than the Dremel equivalent. This is not a machine shop tool so don't expect that kind of precision. The runout does get worse if you try to chuck a shaft smaller than 1/8" and continues to deteriorate with smaller tool shaft sizes.
